New Patients

We welcome patients residing in our practice boundary. Please use the search box by using your post code to see if you are within our catchment area.

If you would like to register with the practice, please download the application form and bring it completed to the practice along with Proof of ID and Address proof.

Registering with the Practice

Any persons within the practice boundary wishing to register with the practice should report to the reception desk. An appointment will be given to complete a new patient medical. Incomplete forms or if we need to contact you for more information may delay your registration.

Please use the links below to complete correct form. Please make sure any parts of the form not applicable please indicate N/A. You will need to provide proof of ID and proof of address. If you are on any repeat medication then please also provide proof of all repeat medication.

For children under the age of 5 please provide proof of immunisations (red book). We as a General Health Practitioner strongly supports the immunisations programmes as a key intervention for improving population health.

Patient Proxy

If a patient does not have sufficient capacity, a 3rd party (family member, friend, carer) can serve as a proxy to assist in the making of decisions on the patient’s behalf.
